Here a 3mm Corneo-scleral incision is given.Here a instrument is used which has three port One,for entry of fluid like Ringerlactate Other port containing a vibrating needle(40000/sec)which brake the opaque lens into pieces and through third port that broken lens is aspirated out. Then a new IOL is placed. As the incision is very small the wound cures in few days.

Now a days a new technique of Laser phaco is done.
